
Family Nexus @ Choa Chu Kang offers holistic and integrated support by providing families and children with easy access to healthcare, social and community services as well as family support programmes within the heartlands.
Our nurses and care coordinators counsel and advise on the Family and Child Health services available at the National University Polyclinics as well as provide the required referrals to our polyclinics, hospitals (specialist services), social services, parent support groups and Child Development Unit, to suit your needs.
